Visitor Access Procedures
For Non-Government or Contractors: This Personal Data Questionnaire (380-2 Form) is required for admittance onto the Depot Base for the Showcase. A form is to be filled out by EACH person coming to the Showcase. The 380-2 Form needs two signatures (attendee and their security/personnel officer) to be accepted by Anniston.
Fax the completed form(s) to Anniston Protocol Office (256) 235-4473 before January 15, 2013.
You will not be allowed entrance to the Base or the Showcase unless you have completed this step by the above date.
For DoD Personnel or Contractors with JPAS access: A visit request through JPAS (Joint Personnel Adjudication System) is required.
